Our Services

Pulaski County Group Home provides a range of residential services for developmentally disabled adults receiving DMH services. Through Medicaid waiver funding, our team of Direct Support Professionals, House Managers, and Program Managers are available 24/7 to support individuals in alignment with their Individualized Support Plans (ISP), including:

  • Safety
  • Transportation
  • Community inclusion
  • Budgeting and finance
  • Cooking
  • Cleaning
  • Activities of Daily Living (ADLs)

Group Home Program

Our group home offers 24-hour, community-based residential care in a co-ed, home-like setting. Residents have access to shared living spaces, private furnished bedrooms, and gender-separated bathrooms. Family-style meals are prepared by staff with respect for dietary needs and cultural preferences.

  • Personal care and hygiene (ADLs)
  • Medication and health monitoring
  • Meal preparation and nutrition support
  • Social and behavioral development
  • Household responsibilities
  • Transportation and community engagement
  • Coordination with healthcare providers and case managers

Independent Supported Living (ISL)

The ISL program offers the same high level of 24-hour support in a more independent living arrangement. Individuals live in their own homes while receiving assistance with daily routines, community access, and personal goals—guided by their ISP and supported by our trained staff.

Respite Care

PCGH also provides short-term respite care for individuals whose primary caregivers are unavailable. Respite residents receive the same level of personalized, compassionate care as long-term residents, with access to all core services and supports.
